Is it ok to top off the carboy with boiled cooled water after racking
to the secondary? Or is this considered "stretching" the brew?

You can, but unless you are trying to hit a specific gravity I wouldn't
recommend it.

yes it would strestch the beer but it would be quite watered down depending
on how much water you have to add. Personally my brews tend to come up shy
of 5 gallons at bottling time but I prefer quality over quantity. Just my 2
cents.

So far, I live with what I get, then try to adjust on the next
brew. I'm getting closer.

IIRC, some industrial breweries add water to finished beer.

My preference would be to add the water at the start of
fermentation. I think this is fairly common in home brewing, if
the wort is sweeter than planned and the brewer is trying to hit
a style.

For some reason, it seems more "kosher" to adjust original
gravity than final gravity.

You can, but why would you want to? You're just going to be watering
down a perfectly good beer (unless you never had enough water in the
first place).
